概要
問題と目的
小児不安障害は学業機能を妨げる
例えば分離不安の移動は朝に強い不安を示す
小学校の教師は、親から離れた後で落ち着くようサポートし、教室にとどまり活動するようサポートする
日中に親に電話したいと要望したり、授業を中断したり、自分や他人の学習を妨げる可能性がある
全般性不安障害の生徒にって、学業の溶融はパフォーマンスと完全性への不安を生む
間違いや教師から失望される恐怖によって、教師からの保証をもとめる
社交不安障害の生徒は、批判を恐れて回答や質問を回避しやすいため、教師に助けを求めたりグループ活動をのぞむ
教師の役割は教育だが、くわえて、不安のような精神商用の緩和も役割の一つである
メタ分析では適切な訓練を受ければ教師は生徒にぽいてぃぶな影響を及ぼすユニバーサルなコーチングを行うことができる (Durak et al. 2011; Anticich et al., 2013; Barrett et al., 2005; Barrett & Turner, 2001; Miller et al. 2011; Skyabina et al., 2016; Stallard et al. 2014)
生徒の不安の緩和は学業成績向上につながる (Weems et al., 2009; Wood, 2006)
しかし、エビデンスに基づく生徒の不安に関する介入の訓練をうける教師はほとんどいない (Reinke et al., 2011)
そこで、教師に生徒の不安に介入する方法を訓練するプログラムを開発した (TAPES; Teacher Anxiety Program for Elementary Students)
方法
参加者
40名の小学校教師
60名の小学生 (包含基準は、小学生、Spence Child Anxety ScaleのT scoreが60以上、もしくはDSM-5不安障害の重症度が3以上;除外基準は自殺企図歴があるなど精神科・医療の提供を受ける必要がある場合は除外した)
介入条件
TAPES
中核的なコンポーネントはCBTである (Cartwright-Hatton et al. 2004; Ginsburg et al., 2011; James et al, 2013)
教師によって学校で各家族に個別に提供される
8週間で5回の各30分間のミーティングを含む (初回のミーティングは教師と家族のみ)
(もうこの時点で日本で実施できない)
必要に応じて電話や電子メールによる教師と保護者の連絡でミーティングを補える
学校と家庭の交流の重要性は強調されており双方の要素を取り入れた心理社会的介入は学校成績と行動の改善に成功している
また、教師が報告する両親との関係改善が児童のアウトカムの媒介要因となる
(親との関係が重要となる)
ミーティング1:不安に気づく (親とのミーティング)
教師は親に理論、ゴール、アウトカムを伝える
マニュアルに従って教師は認知行動療法お出るに従い親へ不安に関する心理教育を行う (なんでだろう、専門職が行えばいいのに)
さらに認知行動療法モデルにおける不安の減弱モデルを提示して生徒に教えるべきスキルを予測する
エクスポージャと、報酬による行動強化に重点を置く
生徒の不安を増長させている行動を振り返り、減少させる行動んに置き換える (教師とともに行うことに意味があるのかもしれない、先生はやってくれないのに親に押し付けるのかという不満も少ないかもしれない)
ミーティング2:不安に気づき不安減弱スキルを学ぶ
教師は生徒にTAPESのオリエン
生徒は不安の三兆候と自分の不安のサインに気づく練習をする
CBTモデルに基づく不安リダクション法のレビュー
自分の不安のサインに気づくためのジャーナリングを行う
教師によってリラクゼーション・呼吸法・漸進的筋弛緩法が提供される
(親と協同するCBTを目的にしているとわかる)
ミーティング3:恐怖に向き合う
教師は不安の兆候を確認し、恐怖に直面する理由を説明
生徒の学校・家庭の回避にもとづきエクスポージャーリストを作成
ミーティング4:コーピング思考
回避行動や意欲に関して振り返り
認知的ゆがみや (これはわけわからないので保留)、非適応的な思考に打ち勝って置き換えるコーピング思考を生徒・親を交えて協議
エクスポージャ計画
ミーティング5:将来の不安へのプランとコーピング計画
生徒・親・教師で振り返り将来のプランを作成
教室の要素:
情緒的にサポーティブな環境が不安を減弱する (Gazelle, 2006)
各教師には、生徒間の向社会的な交流を増やし、チームワークと協力を奨励し、肯定的な行動が一貫して行われるための戦略も提供される
関係性の要素:
リサーチによれば、例えば問題を頻繁に指摘するようなコントロールする教師の態度は児童の不安やネガティブな感情を増やして学習能力を損なう (Assor et al., 2005)
生徒の不安を増長・緩和する行動に気づいて変えられるよう努める
また、教師生徒のポジティブな関係は行動障害 (社会的問題行動を指している) の保護因子となる (O'Connor et al., 2011)
したがって、RELATEスキルによる関係改善に努める (Respect, Empathy, Listening, Acnowledging efforts, Teamwork, and Encouragement)
教師の行動変容理論
上図のモデルは、メンタルヘルス介入の適応における教師の行動変容に関するHan and Weiss (2005)、Guskey (2002) のモデルに依拠している
コンポーネントは、変えられない要素 (州、国、学校など) とパフォーマンスへのフィードバックのような変容可能な要素に分類される
ほかに、教師が新たなスキルを活用したり適応するための要因がある (Tschannen-Moran & Hoy, 2001)
例えば、教師の効力感 (Stein & Wang, 1988)、バーンアウト態度や生徒とのインタラクションに相異や敵意を生むようなネガティブな要素 (Maslach et al., 1996) も含まれる
教師の新たなスキルの使用可能性の感覚も行動変容に影響する (Reimers et al., 1987)
特に、新たなスキルの理解と、それが生徒の行動変容やサポートに役立つという信念が重要 (Kealey et al., 2000; Griffin & Barnes, 1984; Ward & Tikunoff, 1982)
生徒の行動変容に関する理論
不安の病因的なモデルは、過度の生理学的な覚醒、認知的な歪み、恐怖刺激への回避が関与する (Barlow, 2004)
TAPESのCBTモデルの基盤は、過覚醒、不適応な認知、恐怖刺激への回避の変化が不安を緩和して学業成績を向上させるとするもの
不安が学業成績に及ぼす影響に関する研究はあまり行われていないが、過度な不安が生理学的な覚醒を促し、授業への注意をそらして恐怖刺激への注意を高め、ワーキングメモリをさまたげ、児童の記憶の早期を妨げると考えられている (Ma, 1999; Owens et al., 2008)
実際に、不安が児童のワーキングメモリを妨げることは示されている (Owens et al., 2008)
TAPESの理論は、児童のCBTのデータが裏付けている (Higa-McMillan et al., 2016)
教師の行動が修正され、学校と家庭でのスキルが用いられるようになると、教師と親の関係が改善され、児童にポジティブなアウトカムをもたらすと予想される (Evans et al., 2003)
TAPESトレーニングとコンサルテーション
チームは6時間の個人的なトレーニング (アクティブで実践的な学習、観察の機会、モデリング、ロールプレイ、毎週1回30分のコンサルテーション) を受けた
コンサルテーションは、ケースレビュー、スキルリハーサル、問題解決の戦略、オーディオに基づくセッションのフィードバック
対照群:Teacher Anxiety Training (TAT)

TATは生徒の不安のサイン、原因、経過、効果的な介入で構成される
測定内容
①教師の知識とスキル
知識アセスメントA/Bは25項目、多肢選択式の質問紙で、Aは介入前、Bは介入後
教師のスキルに関する授業の観察評価は、介入前後及びフォローアップに置いて実施される、5件法のリッカート尺度
②TAPESの適応度尺度
School-Home Fidelity and Quality Measureは、研究者によって、セッションのレコーディングに基づいて4件法で評定される。アドヒランスミーティングは、目標達成、クオリティなど
Teacher S-H Meeting Summary form (16-item)は、教師によって、親と生徒の参加、関与、技術使用への障壁、目標達成について7件法で回答するリッカート尺度
③教師と児童のTAPES理論による変化に関する尺度
教師と生徒の関係性尺度 (Pianta, 1992) は15項目の関係性を測定する尺度で、介入前後及びフォローアップで測定
教師と親の関係性尺度 (Vickers & Minke, 1995) は24項目の関係性を測定する尺度で、介入前後及びフォローアップで測定
教師の効力信念尺度 (Tschannen-Moran & Hoy, 2001) は12項目の教室マネジメント及び生徒へのエンゲージメントに関する効力感の尺度で、介入前後及びフォローアップで測定
教師のソーシャルコンテクスト (Belmont et al., 1988) 尺度から、教師の関与・援助尺度を生徒が感じる教師の愛情・調和・献身・信頼性を測定する4件法のリッカート尺度で、介入前後及びフォローアップで測定
教師の背景フォーム (16項目) は、ベースラインで教師の基本属性を測定する
Maslach Burnout Inventory は教師のバーンアウトの強さを測定する22項目の尺度 (Maslach et al., 1996; Iwanicki & Schwab, 1981)
Organizational Readiness Questionnaire (Lehman et al., 2011) は、ベースラインにおける学校の雰囲気を測定する25項目尺度
児童生徒の言語性ワーキングメモリをベースライン測定
Family Accommodation Scale, Anxiety (FASA; Lebowitz et al., 2012) は13項目の家族ストレスへの適応を測定する5件法の尺度であり、親が介入前後及びフォローアップで評価する

School Refusal Questionnaire (Albano & Silverman, 2024) は、要するに登校拒否をそくていするもので、介入前後及びフォローアップ時に親へのインタビューにより評価される
生徒のメンタルヘルス
ADIS-5-C (Albano et al., 2024)は児童の不安障害に関するゴールドスタンダードの評価基準であり、介入前後及びフォローアップ時に親子に実施
Clinical Global Impression Severity (CGI-S; Rush et al., 2009)
The Children's Global Assessment Scale (CGAS; Shaffer et al., 1983) 児童の全般的な機能障害を測定するもので、1-100で評定し、介入前後及びフォローアップ時に評価する
Spence Children's Anxiety Scale, Child and Parent Versions (SCAS) は、不安症状が生起する頻度を評価する38項目4件法の尺度で、介入前後及びフォローアップ時に親及び生徒が評価する
Strength and Differences Questionnaire Teacher Version (Goodman, 1997) は、教師が評定する児童の問題行動に関する25項目3件法のリッカート尺度で、介入前後及びフォローアップ時に評価
The Teacher Observation of Classroom Adaptation Checklist, Concentration Problems scale (TOCA-C; Koth et al., 2009) は、教室内での不注意行動を測定する、7項目の尺度で、教師が介入前後及びフォローアップ時に評価
回避の階層は、生徒が回避する上位3場面について、回避する頻度を7件法で測定するもので、介入前後及びフォローアップ時に評価する
