嚥下障害を簡単に⑤咽頭編

前回は咽頭の食べ物を用いない評価について書きました。今回は、経口摂取時の咽頭の評価について簡単なポイントを書いていきます。
経口摂取した際の①嚥下反射の有無、②湿性嗄声、③咽頭ゴロ音、④ムセの有無を確認します。評価のポイントは、前回書いた食品を用いない評価のポイントにも似ていますね。

①嚥下反射の有無
食べた時に嚥下反射の有無は重要です。可能であれば喉に指をあてて、喉が挙上しているか確認しましょう。時々、口の中をみて、なければ飲み込んでいると思われる方もいますが、咽頭に流入しているだけの可能性があるので注意が必要です💦

②湿性嗄声の有無
食べている時に湿性嗄声がるあると、咽頭に残っている事や誤嚥の可能性があります。痰が多い方は分かりにくい事もありますが、湿性嗄声が強くなっていないか確認してみましょう。

③咽頭ゴロ音の有無
咽頭ゴロ音も②湿性嗄声と同様で食べ物が咽頭に残っている事や誤嚥の可能性があります。頸部聴診で確認するとより分かりやすいです。

④ムセの有無
ムセの有無は最も分かりやすい評価ですね。しかし、しっかり喀出できていれば誤嚥のリスクは少ないと思われますので、ムセているからと安易に絶食にせずに、ムセて喀出できているかしっかり確認しましょう🙆

In the previous article, I wrote about evaluation of the pharynx without food.In this article, I will write brief points about evaluation of the pharynx during oral intake.
We will check for the presence of (1) swallowing reflex, (2) wet hoarseness, (3) pharyngeal rattling sound, and (4) choking when the patient is eating food .The points of evaluation are similar to the points of evaluation without food written in the previous section.

(1) Presence or absence of swallowing reflex
The presence or absence of a swallowing reflex when eating is important.If possible, put your finger on the throat and check if the throat is elevated.Sometimes people look in their mouth and think they are swallowing if they are not, but be aware that it may just be an influx into the pharynx💦.

(2) Wet hoarseness
If there is wet hoarseness while eating, there is a possibility of remaining in the pharynx or aspiration.It is sometimes difficult to tell if the person has a lot of phlegm, but check to see if the wet hoarseness is louder.

(3) Presence or absence of pharyngeal rattling sound
As with (2) wet hoarseness, there is a possibility of food remaining in the pharynx or aspiration.It is easier to understand if it is confirmed by a cervical auscultation.

(4) Presence or absence of choking
The presence or absence of choking is the most straightforward evaluation.However, if the patient is able to extract well , the risk of aspiration is considered to be small.so don't simply fast because the patient is choking , you shoud make sure choking and extraction well.
