医療と介護の一丁目一番地は連携です。連携はICTが最も真価を発揮する場です。デジタル技術を活用することで、医療と介護の連携を強化し、PFMを強化することで300床クラスの病院で年間収益を16億円改善させた事例もあります。
医療DXの原理原則は、『医療に関連するすべての生活がこれまで存在していた制約から ICT によって解放され、医療の在り方を再定義し、これまでにない価値創造を実現する未来と出会うこと』であると考えます。私たちは、現場の課題や目標達成に向けてデジタル技術を活用した新しい価値を提供するプラットフォーム「Pubcare」を産み出しました。プラットフォームを構築するにあたり私たちは「Connextion by Pubcare」というフィロソフィーを掲げています。
厚生労働省は令和5年6月に医療DXの工程表を発出しました。医療DXは、医療現場の課題を真に理解し、その中心にいる医療者にとって真に有益なものでなくては成功しません。医療現場のワークフローが、医療経営も含めた様々な課題を解決するBest practiceとして制度設計される必要があります。
医療DXにおける制度設計のキーサクセスファクターはPHRをEHRにすることにあると考えています。

我々は３つのサービスラインでプラットフォームを構築しています。
医療や介護施設とSNSやPHRで連携するアプリPubcare app。オンライン診療を中心に、ビデオ通話機能を有するPubcare online。PHRを起点にしEHRとして相互連携・共同編集ができるシステムPubcare proです。
PHRは医療者が入力することでその質が担保されます。
医療者は様々な帳票を作成する過程でデータを入力し、他の医療者とそれを共同で利活用することで、重複業務を大幅に削減でき常に最新の患者情報にアップデートされていきます。いわば患者さんのwikipediaを共同編集するようなものです。
Pubcareは少しずつ病院に浸透しつつあります。利用者が増えることによりプラットフォームによるネットワーク効果が生まれていきます。データのフローとストックはPHRをEHRとして医療者に生産性の向上をもたらし、課題解決に留まらずストックされたデータからはRWDが得られ、これからの新しい医療の在り方の柱となるだけでなく、ヘルスケア領域にとどまらない新たな価値や未来を創造してゆくことができます。

The fundamental principle of Medical Digital Transformation (DX) is the liberation of all aspects of life related to healthcare from previous constraints through ICT, redefining the way healthcare operates and realizing unprecedented value creation in the future. The Ministry of Health, Labour and Welfare issued a roadmap for Medical DX in June of Reiwa 5 (2023). Success in Medical DX requires a deep understanding of the challenges in healthcare settings and the delivery of solutions that truly benefit healthcare professionals. The workflow in healthcare settings, including healthcare management, needs to be designed as a Best Practice to solve various challenges.
A key success factor in the design of healthcare DX systems is the transition from Personal Health Records (PHR) to Electronic Health Records (EHR).
Our services are distinguished by their comprehensive and unified nature. We have built our platform across three service lines:
Pubcare app, which facilitates collaboration between healthcare and caregiving facilities with social networking services (SNS) and PHR integration.
Pubcare online, centered around online medical consultations with video call capabilities.
Pubcare pro, a system that allows mutual collaboration and co-editing starting from PHR and extending to EHR.
PHR's quality is guaranteed through input by healthcare professionals. Healthcare professionals input data during various report generation processes, and by collaborating with other healthcare professionals, they significantly reduce redundant tasks and continuously update patient information. It can be likened to co-editing a Wikipedia page for a patient.
Pubcare is gradually gaining traction in hospitals. With an increasing number of users, the platform generates network effects. The flow and storage of data, transitioning PHR to EHR, enhance productivity for healthcare professionals. This not only addresses challenges but also extracts Real-World Data (RWD) from stored data. It becomes not only a pillar for the future of healthcare but also creates new value and a future that extends beyond the healthcare sector.
