第７ 環境モニタリング
1        目的
環境モニタリングの一般的な目的は、環境の清浄度を維持する上で、重要区域及びその他支援区域において、微生物数及び浮遊微粒子数が要求される基準を超えないよう管理すること、環境の悪化を事前に把握し最終調製物の汚染を防ぐこと、及び清浄度維持のための清浄化及び殺菌又は消毒の効果を継続的に評価することにある。
＜具体例＞
患者自己免疫細胞の調製においては、一般的にロットが構成されず、通常は投与単位ごとの全数に無菌試験が設定され、最終調製物あるいは工程内に設定された直近の無菌試験の結果を踏まえて投与の可否が判定される。このような場合にあっては、最終調製物の無菌性保証を目的とした環境モニタリングが必ずしも適当であるとは言えない場合も考えられる。
例えば、そのような場合の環境モニタリングは環境の悪化を事前に検知すること、及び清浄化及び消毒方法の効果の評価を主な目的として実施することも考慮されるべきである。
環境モニタリングは、微生物管理と浮遊微粒子管理の二つに分けられ、微生物管理は、環境に存在する全ての微生物を解明することではなく、必要に応じた環境維持操作（消毒等）を行うことを目的としている。
また、患者自己免疫細胞を取り扱う施設であることから、交差汚染については厳密な管理が求められるため、環境モニタリングに係る方法や手順が、封じ込め対策よりも優先されないように考慮すべきである。

＜具体例＞
重要区域において浮遊微粒子測定のために、エアロゾルの発生が想定される作業区域に測定プローブを設置することは、プローブを介した交差汚染のリスクが想定される。また、測定器に吸引した空気を完全性が保証されたフィルターでろ過するなどの適切な処理を施さずに封じ込め装置から外へ排出することや、封じ込め装置としての性能保証を損なう改造等は避けなければならない。
2        一般要求事項
1)       適用
環境モニタリングは、重要区域（グレードＡ）及び直接支援区域(グレードＢ)並びにその区域に隣接する、その他の支援区域（グレードＣ及びグレードＤ）に適用する。ただしグレードＤについては本ガイドラインを参考にして、必要に応じて適用すること。
2)       環境モニタリングプログラム
環境モニタリングプログラム及び実施するための手順を予め定めること。また実施に当たって適切な記録が作成されるようにすること。

7 Environmental monitoring
1        1 Objectives
In maintaining the cleanliness of the environment, the general objectives of environmental monitoring are to control the number of microorganisms and suspended particulates in critical areas and other support areas so that they do not exceed the required standards, identify environmental degradation in advance and prevent contamination of final preparations, and to continuously evaluate the effectiveness of cleaning and disinfection to maintain cleanliness.
<Specific example>
In the preparation of personalized immune cells, generally, the final products do not form a lot, and the sterility test is usually set for all of the units of administration. The most recent results of the sterility test set in the process or for the final preparation are used to determine if administration is possible. In such cases, environmental monitoring for the purpose of assuring the sterility of the final preparation may not always be appropriate.
For example, environmental monitoring in such cases should be considered for the primary purpose of detecting environmental degradation in advance and evaluating the effectiveness of cleanliness and disinfection methods.
Environmental monitoring can be divided into two categories: microbiological control and suspended particulate control. Microbiological control is not intended to elucidate all microorganisms present in the environment, but rather to perform environmental maintenance operations (e.g., disinfection) as necessary.
In addition, since in the facility that handles personalized immune cells, strict controls are required for cross-contamination, and consideration should be given to ensure that methods and procedures related to environmental monitoring do not take precedence over containment measures.
Records related to environmental monitoring should be created and stored in accordance with Chapter 4, Section 14.
<Specific example>
In order to measure suspended particulates in critical areas, measurement probes need be installed in work areas where aerosols are expected to be generated, thus the risk of cross-contamination through the measurement probes should be assumed. In addition, air aspirated into the measuring device must not be discharged out of the containment device without appropriate treatment, such as filtering with a filter validated integrity, and modifications that impair the performance guarantee as a containment device must be avoided.
2        General requirements
1)       Scopes
Environmental monitoring applies to critical areas (Grade A) and direct support areas (Grade B), as well as other support areas (Grade C and Grade D) adjacent to these areas. However, Grade D shall be applied as necessary, referring to this Guideline.
2)       Environmental monitoring program
An environmental monitoring program and procedures for implementation shall be established in advance. Appropriate records shall be created for implementation.
