people sharing the same name：同姓同名の人。他にもnamesake、a person with the same family and personal nameなどと表せる。

effectとaffectの違い

affect は動詞として使われることが多く、「〜に影響をおよぼす」とか「〜に作用する」という意味になる。effect は名詞として使われることが多く、「効果」「結果」「影響」という意味になる。

“this note will not take effect unless the writer has the person’s face in their mind when writing his/her name. therefore, people sharing the same name will not be affected.”：書く人物の顔が頭に入ってないと効果はない。ゆえに同姓同名の人物に一遍に効果は得られない

cause of death：死因

specify：を明細に述べる

a heart attack：心臓麻痺。個々の事例は加算名詞。主語がpersonなので加算なのだろう（症状としては不加算）

“if the cause of death is not specified, the person will simply die of a heart attack.”：死因を書かなければ全てが心臓麻痺になる
