USMEL step1申込み手順（2019年GW対策開始、2020年3月合格）

USMLE step1は基礎医学試験、FIRST AIDとUworldで対策を

2019年から2022年にかけての私の経験談です。適宜変化しているので、最新情報をUSMLEとFSMB(Federation of State medical boards)で確認すること。

USの医学生は4年間の在学中にUSMLE（United State Medical Licensing Examination） Step1, Step2CK, Step2 CS（COVID19後2019年春頃から中断中）に合格が必要　。IMG(International Medical Graduate)はECFMG(Educational Commission for Foreign Medical Graduates)を通じて試験の申込みをし、Step1/2に合格すれば、ECFMG Certificateが得られる。ECFMG Certificateが得られれば、米国で指導医の元臨床ができる。レジデンシー中にStep3に合格するか、Step3の合格をしてからレジデンシー開始してもよい。Step3合格の有無で、J1visaかH1Bvisaか、発行されるVisaの違いがある。日本と違い、医師免許は州毎に発行される。日本と違い、医師免許は州毎に発行される。最後に、自分で働く州の医師免許委員会に免許発行を申請する必要がある。

この３つの試験はどの順で受けてよく、Step2 CS以外はCBT(computer-based test)なので世界各国で受験可能です。日本では東京、御茶ノ水ソラシティか、大阪中津センタービルのPROMETRIC　のPROMETRICが会場です。

USMLE step1申込み手順

  1.  ECFMGサイトに登録する　　125ドル

  2. NortaryをしてECFMGに提出する（Form186）

  3. ECFMGのOASISから、Applicationを提出して、試験の予約をする　2019/1/19　OASISからStep1の申し込みを行った。1305ドル。　

  4. 大学の卒業証明（Form 183）に記載し、卒業大学から直接ECFMGに郵送を依頼
    　　英文証明に関しての注意点

  5. Schejuling Permitが発行され、試験日を予約する　　　　　　　　　　2019/4/4　Your USMLE® Step 1 scheduling permitがメールで届いた

  6. １回のみ予約期間を変更することも可能

１．ECFMGサイトに登録する


まずECFMGのTop PageからOASIS(On-line Applicant Status and Information ystem)に行き、「Go to OASIS」をクリックし、まずは自分の情報を登録する。2018/10/6　ECFMGに登録
　　2018/10/15　ECFMGから登録完了のメール受け取る
　　2018/11/16　ECFMG　On-line Servicesで本登録をする。最後の登録料125ドル(2019年当時）を払って終了。次にNortaryが必要で、今はオンラインですぐに可。

２．NortaryをしてECFMGに提出する（Form186）

2019/1/4　Form186をNortarycamで公証してもらいECFMGに提出
2019/1/16　ECFMGからメールで自分のIDナンバー通知
2019/1/19　Your Certification of Identification Form submittedのメール

　Nortaryとは公証人という意味で、署名が本当に本人のものか、証明する作業になります。アメリカ国内であれば、病院内でも、公的な郵便局や銀行、UPS（宅配便）などの店舗でもすぐに対応してもらえますが、日本ではそういうわけには行きません。公証人役場やアメリカ大使館でのNortaryがもともと必要でしたが（Step3は2022年1月時点では公証人役場、大使館などでのNortaryが必要だった）、Step1の申込みに関してはオンライン24時間サービスであるNortaryCamで、あっという間に行ってくれます。

ECFMGの書類には以下の記載があります
「To use NotaryCam to complete Form 186, 」（Form186 はStep1申込みを行うとメールからダウンロード可能になる書類で、あなたが本人であるとの証明。日本で置き換えると戸籍抄本とか住民票みたいなものか）。これに公証人に書類のサインがあなた本人のものである、と証明してもらうのです。

you will need:　必要なものは

  1. PDF file of Form 186 ECFMG provided to you　（ECFMGのHPからダウンロード可能となる）

  2.  a scanned, color image of the required page(s) of your passport

  3.  a computer with a webcam　（NortaryCamの人と顔を見ながらビデオ通話する必要があるため)　

Form186の説明

NortaryCamのHPに行き、サインアップ、自分のDashbordができるので、ECFMGからもらったForm186、自分の顔写真をUploadすると、すぐに反応があり、今からMeeting可能か連絡が来た。OKと返事をすると、Google Chromeを使うように言われ、Weblinkが送られてくる。アクセスすると、PDFに写真を貼り付けたものがDashbordに現れる。しばらく待つ。と、右上のところに「Join video conference」の表示がでて、クリックすると、Webcamで相手の公証人とオンラインでつながる。
そこで名前、生年月日、住所などを聞かれるので、英語で答える。パスポートのページ見せて、と言われるので、パスポートの写真ページを見せる。
そしたら、写真に公証人のスタンプ押したForm186に切り替わる
書類の左下を見るように言われると、いくつかのTabが出てきてて、Add signatureを押すように言われる。Tabをクリックすると4つくらいのフォントで自分の名前が出てくるので好きなフォントを選ぶとサインが入る。
しかし、その時気づいたが、名前が一文字間違ってる！！！私の打ち間違い？よくわからないけど、すぐにメールすると再度Video conferenceボタンが現れ、左上にPick signatureのボタンをクリックすると、再度Signatureが出ので、修正。そしてまた4つのFontから好きなものを選んでサイン。完了！
あー、危なかった～。ECFMGにはNortarCamから直接サイズ身書類が送付され、Updateされたら、本人にメールが来るとこのこと、10 buissiness daysくらいかかると言われた。良かった、無事終わって。

３．いよいよStep1のApplicationを提出

ECFMGのOn-line servicesのIWA（ECFMG's Interactive Web Applications）にアクセス

  • Time Limit for Completing Examinationは　7年。最初の試験を受けた日から7年以内に、すべてのテストを終えなければいけない。

  • You must submit a copy of your medical diploma at the time of exam application if your diploma has not been sent to ECFMG previously.

All documents that are not in English must be accompanied by an official translation that meets ECFMG’s translation requirements. See English Translations in Medical Education Credentials.
卒業した医学部の卒業証明書の原本と英訳が必要。大学からも英訳したものを送ってもらえるのですが、これはECFMGの認めるOfficialな英訳はなく、結局別のサービス、Strakerに頼む必要があった。

次にECFMGのOn-Line Servicesで　”Application for USMLE　Examinations”にアクセス。3か月毎に受験日を設定するEligibility Periodを選ぶので、選択する。

この下に３ヶ月毎の期間が選択できるようになっている。
次は場所。日本だけ何故か高く。365ドル（2019年当時）

  • dates of  attendance の title of medical degreeを’Igaku’`と書くというネット上の情報に従い、Igaku（Bachelor of Medicine)と記入。

  • clinical clerkships　この欄には何も記入せず

  • 最終的な申込画面で＄1,305支払い（Examination fee $940, International Test Delivery Surcharge $365)

これで申し込み終了。受からないともったいない。

４．大学の卒業証明（Form 183）に記載し
卒業大学から直接ECFMGに郵送を依頼

(最初にNortaryしたのはForm186なので別物ですよ。)
上記のアプリケーションを送信したときに、卒業証明も提出するように求められる。この証明書類がForm183です。
レターサイズ（米国のみ謎に使われている用紙サイズ。世界的にはA4 だけど、ちょっと違う）で、余白を2.5㎜に設定するときれいに印刷できた。ネット情報では右上に写真のスペースがあるけど、最新（2019年時点）のものはないようだ。自分のサインと、日付を書いて、後は卒業大学に送って、書いてもらう。
直接大学から郵送か、最近はwebPortalからでもオンライン送信できると書いている。書類について事前に問い合わせしてみたが、私の卒業大学ではWebPortalはまだ使うことはできないといわれ、郵送してもらうことにする。
そのほかの自分で送る書類はMobile appでアップロードできるようになっていたので、とりあえず、IpadにMobile appをインストールし、まずは自分の学位記（卒業証書）を写真とってアップロード。簡単簡単。

Instructions for Certification Statement (Form 183)
- Print a copy of the Certification Statement (Form 183) by clicking your web browser's "Print" button. If a portion of the Certification Statement (Form 183) is cut off on the copy that you print, you will need to adjust the margins under "Page Setup" in your browser. Generally, if you set all margins in your browser to 0.25 inches or 2.5 millimeters, the copy of the Certification Statement (Form 183) you print will be complete.

- Sign and date the Certification Statement (Form 183). The medical school official must sign and date the Certification Statement (Form 183) and provide his/her name, official title, and the institution name. The official must affix the institution's seal in the designated section of the Certification Statement (Form 183). Each medical school has been requested to provide ECFMG with a list of the officials authorized by the school to certify Form 183. Your Certification Statement (Form 183) must be certified by an official on this list. Contact your medical school to determine the officials authorized by your medical school for this purpose; ECFMG will not provide this information to applicants. If your form is not signed by an authorized official, it will not be accepted. The signature of the official must be current; the official must have signed the Certification Statement (Form 183) within four months of the date that it is received at ECFMG. The form must be sent to ECFMG directly from the office of the medical school official who certified the form. If the form is not sent to ECFMG from the office of the medical school official, it will not be accepted.

- The following documents must also be submitted to ECFMG:

1. 卒業生のみのものGraduates only: A copy of medical diploma (if not previously submitted) and official English translation of medical diploma (if diploma is not in English).

2. If you transferred credits to the medical school that awarded or will award your medical degree, a copy of an official transcript issued by any school or institution from which you transferred credits to the medical school that awarded or will award your medical degree (if not previously submitted); and official English translation of transcript (if transcript is not in English.) For information on ECFMG's translation requirements and recommended translation vendor, see English Translations in the ECFMG Information Booklet. 3. Any additional documents required to complete your application.

2019/1/20　またECFMGからメール来た

Dear Doctor:

We are pleased to confirm that the On-Line part of your USMLE Step 1 application (Application Identification CodeXXXX) has been received by ECFMG. Your USMLE/ECFMG Identification Number is listed in the Subject line of this message. You must use this number in all communications with ECFMG.

Once your application has been reviewed, ECFMG will notify you of the outcome of your application.

ECFMG processes exam applications typically within two weeks of receipt of the on-line part; verification of your student or graduate status from your medical school; and any other required documents. You may track the status of your application by using ECFMG's On-line Applicant Status and Information System (OASIS).

大学に依頼して、直接郵送してもらうのを待つ。

５．Scheduling Permitが発行され、試験日を予約する

2019/4/4　Scheduling Permit発行された　Your USMLE® Step 1 scheduling permit is now available. To access it, log in to ECFMG®'s Interactive Web Applications (IWA) at https://iwa2.ecfmg.org. A copy is also attached for your convenience.

2019/5/1　令和元年　スケジューリングを行う。　Eligibitlity Periodの半年前から予約可となる。
PROMETRICのHPに行って、予約する。Eligibitlity Period内であれば、何回でも予約変更はできます。予約は取りやすかったが、早めに予約すればより自分の都合にあった日付で予約できます。
筆者は仕事が忙しすぎて、一度Eligibitlity　Periodを延長した。そうすると、新たなSchedulling permissionが届くので、それで再度予約を取り直せばOK。ちなみにEligibility periodの延長は1回のみ。それ以外だと、再度最初から申し込みをやり直し、費用ももう1回払う必要があります。

私は、仕事の都合かつかずEligibitlity　Periodの最終日を受験にしたのだけど、コロナ禍真っ最中で、発熱したらもう延長できない、とびくびくしていたので、本当にEligibitlityは余裕もって、その早めの時期に予約するのがよいですよ！

６．１回のみEligibility periodの延長可能

Eligibilityの延長方法がよく分からず、ECFMGのHPに記載の問い合わせるメールした。日本のプロメトリックにも電話したけど、Eligibility内の日付変更は日本のPrometricでできるけど、Eligibilityそのものの変更はECFMGに問い合わせなければならない、との返答。
ECFMGからは、「３日以内に返事」と、すぐリプライがあったが、実際はbuisiness daysで土曜日に送って、返事は翌週木曜日。しかし、メールだけのやり取りで、無事に延長できた。８０ドルで１回のみの延長が認められている。
さらに延長のときは、いったんキャンセルして、全部やり直しだそう。
