26 y.o Male No physical Trauma, History of Hitting Head, Diarrhea, Hair loss, Purpura

 At the time of the bombing, his head was pressed with concrete wall for 30 minutes. He recovered after that. When he was going to Kyoto by train, he felt sick on the way and came back.

On the 16th day, he was taken to Saijo clinic. He did not have any burns or trauma. At that time, the number of white blood cells was as low as 1500 and the number of red blood cells was low at 2.97 million. It was indicated that he was in the state of anemia and immune disfunction.

Day 17: Diarrhea.

Day 18: Marked hair loss.

Day 19: Purpura (internal hemorrhage) on the whole body was seen.  At 8:30 am, the patient's speech became incomprehensible, and he had high fever at 39℃. He passed away at 17:50.

The Findings of Autopsy


Because his remarks became incomprehensible before his death, the brain findings should be focused on investigation.But unfortunately, the record of his brain was missing or lack.

On the other hand,  severe necrotizing colitis (colitis with ulcer), laryngo-pharyngitis, necrotic tonsillitis, and bleeding findings (pharynx, epicardium, pleura, renal pelvis) were observed.


Explanation:


It is speculated that the bleeding tendency seen on the pathological autopsy findings was affected by radiation throughout the body.

One of the causes of death is considered to be infection due to leukopenia.  Because the immune system was weakened, pathogenic bacteria traveled through blood vessels throughout the body. The patient finally had septic shock and multi-organ failure, which could be one of causes of the death.

As another cause of death, the brain should be considered, because this 26 y.o man was not able to make comprehensible conversation. It is possible that intracranial hemorrhage occurred on the 19th day with a decrease in platelets. Although it is not known to be associated with the past head hitting history at the time of exposure, it is possible that the blood vessel damaged by bruising caused rebleeding due to the tendency of bleeding caused by radiation injury.

Alternatively, it cannot be denied that the brain may be infected with pathogenic bacteria, causing encephalitis and brain abscess.

Creating an abscess in the bleeding organ


In addition, as a similar case, an American study team reported that there was an anatomical finding that died due to a brain abscess caused by infection of the cerebral hemorrhage.

In the atomic bomb survivors who suffered organ hemorrhage, it is suggested that the immunity disfunction caused by radiation may cause pathogens to be carried in the bloodstream and form an abscess at the bleeding site.
